The Authority are looking for Providers who will work to rapid response requirements to prevent admissions to Hospital, Residential and Nursing Homes where this is not entirely necessary and assist with facilitating Hospital Discharge in a timely and more efficient process. The Authority is working with the Clinical Commissioning Group to align work to the Better Care Fund and the Care Closer to Home programme.
