Developing a new model of care for patients with more activity delivered in an out of hospital setting has to deliver improved patient pathways and provide an alternative to secondary care with greater integrated working between primary care, community services and the social care system, ensuring patients receive their care by the right person, in the right environment and at the right time. A number of initiatives are being undertaken by the CCG to ensure the fundamental systems are in place to drive integrated care at scale and pace across Trafford’s health economy; the procurement of a Risk Stratification solution being a key part of this programme. Trafford CCG requires a dynamic Risk Stratification solution to help gain a greater understanding of the changing health needs of Trafford’s population and to support intelligent commissioning of health services. This will assist with a pro-active health system. The Risk Stratification solution will also help enable care to be delivered differently. The CCG will be able to identify those patients with an increasing risk of hospitalisation or other interventions, including the need for higher levels of social care, and offer appropriate alternative support through greater utilisation of community and primary care services, social services and, potentially, third sector services. Switching the commissioning of services to deliver pro-active services and the co-ordination of care will be substantially supported through the recently commissioned Trafford Care Co-ordination Centre (TCCC) where Risk Stratification will play a central role. The contract will be based upon the standard NHS Terms and Conditions for Services. The contract term will be 60 months. This contract will not be open to other purchasers.
