Short Description: | Lincolnshire County Council (the Council) is undertaking a procurement exercise for its home-based reablement services across Lincolnshire. This service provides time limited reablement support in service users' own homes with the aim of enabling them to remain living independently, including following discharge from hospital or to prevent an avoidable hospital admission. The contract which results from this procurement will be awarded at the end of September 2015 with a two month transition period to coincide with the end of the existing arrangements in November 2015. The new provider will be expected to be able to delver services and meet demand by this date. The contract value for this opportunity is approximately £4m per annum. The procurement will be carried out in line with Section 7 of the Public Contract Regulations 2015 and this contract notice serves as the required call for competition in line with regulation 75.
