The aim is to implement a new Recovery At Home (RAH) contract provided by an experienced home healthcare provider to patients admitted to University Hospital Southampton NHS FT. This service will ensure the inpatient length of stay will be reduced along with the overall spell length of stay for the services provided. It will achieve this by shortening the length of stay for given patients by offering homecare as an alternative to continued hospital stay for a number of days.
Patients admitted with a range of conditions will be discharged into the care of a Recovery at Home team in the community.
The service objectives are:
• Reduce the average “in hospital” length of stay for recovery at home patients
• Reduce the overall Length of stay for patients
• Effectively utilise resources and deliver cost reductions
• Demonstrate innovation and provide a robust model of care
• To provide a world class patient service
