II.2.4) Description of procurement: In Essex there are approximately 30,000 people living with Dementia. Assuming the prevalence remains constant, for Essex as a whole, there will be a 156% increase by 2050 (50,000 people by 2025 and 73,800 by 2050). The primary aim of the service will be to empower and support people living with dementia, and those that care for them to live an active and enriching life and, be actively involved in planning their own care. The service will give people living with dementia the tools to remain independent, and as part of their community, for as long as possible. However, we also recognise dementia is a progressive condition and as such the service needs to be flexible and responsive, taking into account the wide spectrum of needs and challenges people living with dementia face. We want people to have access to a source of support which is able to respond these fluctuating needs and help navigate individuals and families, through their experience of living with dementia.We want the service to provide a single point of access to a multitude of activities and interventions which enable people to live well, manage times of crisis and avoid unnecessary admission to hospital, and or long-term care settings. The service will be key in driving forward the community support available and working to develop capacity in communities which enable people with dementia, their carer’s and wider family network to live well, and as part of their community for as long as possible. The service will work with people living with dementia, and those that care for them to independently access information and support which enables them to live well, whilst developing confidence and knowledge in how to re-engage with the service and draw on further support if and when required. The Community Support Service will build and develop activity in communities that enables people living with dementia to remain as an active member of their community and able to access support that enables them to live well.
