Community health services. Knowsley Clinical Commissioning Group (KCCG) is seeking to re-commission a Borough wide Community CVD Service.
The service will be for KCCG patients and will provide a multidisciplinary community based CVD Service comprised of a consultant led diagnostic, treatment and management service, heart failure service, community cardiac rehabilitation and community stroke rehabilitation. A central co-ordinating function will act as an administrative hub accepting referrals, arranging appointments and co-ordinating patients’ pathways.
Consultant led, multidisciplinary, one stop CVD diagnostic and treatment/management clinics will be provided in a variety of community locations across the Borough. The service will offer diagnosis and treatment at one appointment where clinically appropriate and a wide range of diagnostics will be offered.
Cardiac rehabilitation is currently provided in leisure facilities and programmes will be tailored to individuals needs with a menu based approach expected. Each programme will be expected to deliver the seven essential core components as set out in BACPR Standards and Core Components 2012. Specialist stroke rehabilitation will be predominately delivered in the patients’ own homes and, if appropriate, in the local community. Community re-integration is promoted and psychological support has been identified at patient engagement events as essential.
The stroke service will provide Early Supported Discharge taking referrals from local acute stroke units at St Helens & Knowsley Teaching Hospitals, Aintree University Hospital and Royal Liverpool and Broadgreen University Hospital. In-reach into secondary care is required to undertake initial assessments to support early supported discharge.
Referrals to the diagnostics, treatment and management services will be accepted clinicians including GPs, Practice Nurses Community Matrons and Nurse Clinicians. The service will be available for people aged 18 years of age and above registered with a Knowsley GP. Cardiac rehabilitation will be available to residents of Knowsley and patients registered with a Knowsley GP aged 18 years and older.
Cheshire and Merseyside Commissioning Support Unit (CMCSU) are procuring this service on behalf of KCCG and will facilitate engagement with potential service providers. An initial engagement event will be held on Monday 29th July 2013 at Whiston Primary Care Resource Centre, Old Colliery Road, Prescot, Merseyside L35 3SX. The event will take place from 12-30pm until 15.30pm. Any Providers wishing to attend this event should in the first instance contact Procurement@CMCSU.nhs.uk providing further contact details.
