Southend-on-Sea Borough Council is inviting expressions of interest from a single (or single consortium) Provider for our Healthy Lifestyle Hub.
Southend-on-Sea Borough Council are implementing a comprehensive programme of tailored lifestyle interventions and services for people living in Southend aged 16 years or over.
The purpose of the Programme is to improve the health and wellbeing of the resident population in Southend and it will be delivered by a number of providers across a range of service areas, including:

•Physical Activity Interventions
•Health trainers (including workplace interventions)

The Provider will be responsible for the delivery of the ‘Gateway’ into the Programme, Health Trainer Service and for providing associated services as described in the tender document.
The Provider will deliver an integrated single point of access to health improvement services, including a holistic assessment of an individuals need and signposting or referral into appropriate pathways before following them up to support maintenance of behaviour change. Delivery of the health trainer service will further support improvements to client’s health and wellbeing with a specific focus on those at risk of health inequalities
The Provider shall perform the Service to achieve the following objectives:
•Encourage and enable health and social care professionals and the wider workforce to deliver brief interventions and refer into the Programme.
•Encourage new clients to be referred into the Programme
•Encourage clients to attend the Provider’s review meetings
•Encourage clients to attend and complete their Programme Intervention
•Encourage clients to achieve the Programme Intervention targets
•Encourage continuous improvement of the Programme
•Ensure the timely onward-referral of clients and information
•Encourage businesses to support health and wellbeing of their staff

The contract term will run for 3 years from 1st June 2016 – 31st May 2019 with an option to extend for up to three additional periods of 12 months. The latest expiry date of the contract will be 31st March 2022.
Southend-on-Sea Borough Council welcomes submissions from all organisational types, including the Third Sector.
The annual contract value will be capped at a maximum of no more than £212,000. The maximum total contract value would be a maximum of £1,272,000 (including the possible extensions.)
