         Community health services. Health and social work services. Health services. Guidance services. The HFH Service is a ‘home from hospital’ service which will support clients to discharge from hospital and their return to living at home.
The primary aims of the HFH Service are:
•   To reduce the number of preventable re-admissions to hospital within 30 days of hospital readmissions (PHOF 4.11) and reduced social isolation (PHOF 1.18)
•   To contribute towards reducing the rate of emergency hospital admissions for injuries due to falls in persons aged 65 and over (PHOF 2.24)
•   To facilitate early and safe discharge from hospital
•   Contribute towards reduced length of initial stay and total days of care
•   To reduce or delay the number of clients being admitted to long term care
•   Improve independent living for clients (evaluated using pre and post intervention client feedback of the service)
The Provider will deliver an evidence based program to service users in the community, promoting independence at home through focussing on social inclusion and providing a safe environment for service users, recognising issues that may hinder their independence.
